Understanding the barriers and facilitators for digital health use in chronic care management will help guide a better path forward, research says.

Both patients and providers recognize the benefits digital health has on patient engagement in chronic care management. However, both parties have apprehensions, which one group of researchers says can arm digital health developers reframe implementation plans.

Digital health and mHealth tools have been given widespread praise in recent years as key technologies for chronic care management. Digital health tools can help patients better understand their chronic illnesses, reflect on their own medical records, receive better education, respond to prompts for self-management activities, and monitor condition changes.
