Congress intended to legislate a “permanent” doc fix as part of the Medicare Access and CHIP Reauthorization Act of 2015 (MACRA). However, substantial questions have been raised about the original design and long term sustainability of the MACRA Quality Payment Program (QPP) for both the Merit Based Incentive Payment System (MIPS) and Advanced Alternative Payment Models (APMs).

Specifically, the Medicare Payment Advisory Commission (MedPAC) has recommended that Congress scrap MIPS, while also expressing serious concerns about the bonus payment scheme for physicians participating in Advanced APMs. The recent budget bill signed into law contained a provision to ease requirements in MIPS, and the President’s Budget proposed to remove all reporting requirements for MIPS clinicians, instead relying solely on...
