It’s time for value-based healthcare administration

Imagine an emergency department (ED) on December 26. Like many EDs, staffing ratios have been set by the facility’s administration, and some clinicians have expressed concern they are inadequate. On this particular morning, patients flock to the ED, as they often do following a holiday. In response, the administrator on call activates the hospital’s surge protocol to mobilize patients out of the ED and coax additional staff into it.

Now imagine that a physician makes a medical error during this patient surge. Two months later, that physician will undergo peer review. She will be evaluated by her colleagues, given learning opportunities, and she may even be asked to participate in a performance improvement plan....
