Transplantation is a field that exists to restore health, and organ transplant has been a durable treatment option for patients with end-stage organ disease for more than 40 years. In the United States, the most common diseases that lead to end-stage organ disease are diabetes, hypertension, obesity, and substance use disorder. These chronic conditions share similar risk factors, including an unhealthy diet and physical inactivity; all are linked to socioeconomic status and other social determinants of health (SDOH). Patients from underserved and marginalized groups have disproportionately high incidence of chronic diseases and, despite the miraculous capabilities of modern medicine, suffer from lack of access to primary care, timely diagnosis and treatment, and sometimes even a basic awareness of their own...
