The NCQA published its 2025 ratings of health plans Sept. 16, evaluating plans on nearly 50 assessments of quality outcomes and member experience.

Just 11 plans out of the nearly 1,000 rated received 5-stars. In the commercial market, Kaiser Permanente, Blue Cross and Blue Shield of Massachusetts, Independent Health and UPMC Health Plan received the top rating.

Here are the top-rated commercial plans in each state:
