Any proposal for universal coverage—from Medicare for All, to a public option, to reinforcement and expansion of provisions under the Affordable Care Act (ACA)—faces the challenge of affordability. Affordability, in turn, may well hinge on the health care delivery system redesigning care to eliminate waste. A key question is whether the delivery system can do this, reducing fragmented care while maintaining or improving quality and meeting patient needs and preferences. This post focuses on the key role that integrated delivery systems must play in making universal coverage—in whatever form—viable.
