Cleo will now be providing family caregiving support to Sonder Health Plans’ Medicare Advantage members. This includes connecting them with medical and social support.

Sonder Health Plans, a Georgia-based health plan provider, is partnering with family caregiving company Cleo to support Medicare Advantage members, the companies shared with MedCity News exclusively.

Sonder Health Plan serves 84 Georgia counties, including Macon, Columbus and Savannah. Working with San Francisco-based Cleo will help the insurer manage chronic conditions, address health-related social needs and direct members to the right care.

Cleo identifies which members have risk factors for physical and emotional well-being and provides tailored care plans to address their physical and mental health needs. Members and their families are connected with a guide,...
