Idaho-based Advanced Health Care of Coeur d’Alene had the lowest risk-standardized rate of potentially preventable hospital readmissions 30 days after discharge between Oct. 1, 2021, and Sept. 30, 2023, according to CMS data published March 26.
The national average rate of potentially preventable readmissions for the same period was 10.51.
