The Biden administration is encouraging states to address the health-related social needs (HRSN) of certain Medicaid beneficiaries through Section 1115 waivers, among other means. This endeavor is laudable but problematic in several respects. Rather than encouraging the development of small, short-term, and likely non-durable programs, the administration would do better to seek a more ambitious solution.

Section 1115 of the Social Security Act allows the secretary of health and human services (HHS), at a state’s request, to waive some of the federal requirements that state must meet to participate in Medicaid, or to provide federal Medicaid matching funds for services or populations that normally wouldn’t qualify, or both.
