DocNow, a Bellevue, Wash.-based startup, raised a seed round to expand its platform that provides electronic medical records, helping providers connect patients to care needed after hospital stays.

The amount of funding was not disclosed. The round was led by Waterline Ventures, an investor in early-stage, healthcare tech startups.
