Home-based cancer care platform Reimagine Care announced it is partnering with in-home care provider DispatchHealth to provide virtual-to-home care for oncology patients.
Reimagine Care partners with providers and health plans to coordinate care in the home to avoid unnecessary inpatient visits. Reimagine clinicians can remotely monitor patients, work with oncologists to determine whether patients need additional services and send home care workers to assist.
Denver-based DispatchHealth allows patients to request an in-home urgent care medical visit via telephone or the company’s app or website. The care team includes a physician assistant or nurse practitioner and/or a medical technician. Care...
