A major trend in delivery system transformation is the explosion in Medicare and Medicaid of what we call “advanced primary care,” which comprises a high-touch care model focused on team-based care, smaller panel sizes, modification of clinical workflows to incorporate data insights, and the assumption of substantial two-sided risk.
Advanced primary care organizations, which began operating in the 1980s, have demonstrated tremendous success in managing high-risk patient populations in Medicare Advantage (MA). In the past 10 years, the successes of early pioneers have inspired a crop of new advanced primary care organizations that have grown rapidly with the backing of private health plans and venture capital investments.
