9,000 community referral summaries have been generated for positively screened patients; 6,000 referrals to community service agencies have been created for high-risk screened patients.
Reading Hospital in Berks County, Pennsylvania, realized that addressing social determinants of health – conditions impacted by the places people live, learn, work and play that affect health outcomes – was a priority.
