Introduction

We’ve all heard some version of the axiom: “If you’re not at the table, you’re on the menu.” Lost in the excitement of healthcare AI and a slew of new CMS/CMMI models is the implementation of the 2026 Final Rules.

On a positive note, the rules include a 3.26% increase in the conversion factor (dollar amount CMS pays per RVU). Unfortunately, that’s about where the good news stops. Most of the increase is temporary, stemming from a one-time 2.5% boost that expires on January 1st, 2027.

The remainder of the Final Rules are far less favorable:

  • A -2.5% “efficiency adjustment” affecting procedural and diagnostic specialties

  • Reductions in RVU practice expense allocation in facility settings
