The Agency for Healthcare Research and Quality (AHRQ) recently issued a review of 47 practices that can improve patient safety in various settings.

Despite national attention and successful interventions in recent years, patient safety remains a serious problem, the AHRQ noted in its report, the third edition of Making Healthcare Safer. The agency estimated that in 2017, there were 86 hospital-acquired conditions for every 1000 hospital discharges, a rate that has steadily declined in recent years but still remains “alarmingly high.”
