MVP registered nurses will work on-site at Rochester Regional Health as care managers, providing wrap-around services to MVP customers during their hospital stay and post-discharge

A recent study from the Joint Commission Journal on Quality and Patient Safety found that only between 50 and 60 percent of patients remember key information from recent inpatient stays, and only 50 percent correctly recalled post-discharge treatment plans. To address this problem, Rochester Regional Health in New York is partnering with MVP Health Care, a nonprofit health insurer in New York and Vermont, to launch a care management and engagement program to better serve patients in the hospital and in their transition post-discharge.
