The Social Security Act mandates that all nursing homes participating in the Medicare program—currently more than 14,000—undergo an annual recertification inspection, known as a standard survey, to ensure compliance with federal regulations and the safety and well-being of residents.

The standard survey is a time and resource intensive process. It begins in a state agency office (typically, although not always, part of the state’s department of health) with one to two days of preparatory activities. The survey team, consisting typically of three to five individuals, then travels to the nursing home.
