Evidence increasingly shows that to improve population health and constrain the rate of growth in health care costs, it is necessary to address the underlying social determinants of health. Recognizing this reality, in 2017 the Centers for Medicare and Medicaid Services provided funds to 30 communities throughout the United States for a period of five years as part of its Accountable Communities for Health (ACH) model. The emphasis is on improving screening for social needs such as housing, food insecurity, and transportation assistance; referral to community and social service agencies; and navigation services. Several states such as California, Minnesota, Vermont, and Washington have undertaken similar initiatives using cross-sector partnerships to improve population heath in defined geographic areas.
