A project by Humana to improve health through better management of patient populations will be expanded to 14 U.S. communities, the health insurer said Tuesday.

Humana said Atlanta, Charlotte and Houston are joining 11 other communities that are part of the health insurer’s “Bold Goal” initiative, which uses measures established by the Centers for Disease Control and Prevention to track an individual’s physical and mental “unhealthy days” over a 30-day period. The social determinants of health for these patients are closely watched with patients screened for “food insecurity” and “loneliness.”

“In 2015, Humana announced its Bold Goal, a population health strategy to improve the health of the communities the company serves by 20% by 2020,” the health insurer said....
