More than a decade ago, Congress created the Center for Medicare and Medicaid Innovation (CMMI) to “test innovative payment and service delivery models” in areas where “there are deficits in care leading to poor clinical outcomes or potentially avoidable expenditures.” Amazingly, despite the fact that chronic conditions affect the majority of adults and are widely recognized as one of the biggest drivers of avoidable healthcare spending, CMMI has never created an Alternative Payment Model specifically designed to support effective care for patients with serious chronic conditions such as asthma, COPD, heart disease, inflammatory bowel disease, or rheumatoid arthritis.
