Home-based care—which encompasses a variety of siloed services and policy avenues provided in the home setting—is critical for improving equitable access to health care. This is particularly true for older adults and people who face barriers to accessing care in traditional clinical and institutional settings, such as individuals who are homebound. Home-based care lacks a uniform set of services and differs by coverage or provider but for Medicare beneficiaries generally encompasses, home health, home-based palliative care, home-based acute care, and non-medical support services.
