Eligible Medicaid patients in Oregon have started to receive free air conditioners, fridges, heaters, air purifiers, and portable power supplies to address their health risks related to climate change. Twenty other states have been using Medicaid dollars to cover nonmedical services such as housing and meals, with 16 more states likely to join soon.

Medicaid is mainly paid by federal taxpayers through matching funds. In 2022, more than 70% of the $830.6 billion spending on Medicaid came from federal taxpayers. The more a state spends, the more federal matching funds it receives, creating a perverse incentive for states to snowball Medicaid spending.

The Medicaid program does not distribute any cash for patients to address their own medical or social needs....
