Next year, important changes are coming to a key feature of the ACO Realizing Equity, Access, and Community Health (REACH) model.

Launched by the Centers for Medicare and Medicaid Services (CMS) in January 2023, ACO REACH reflects a strategic direction by the CMS Innovation Center to orient payment around equity goals. In particular, the model includes a number of foundational advances for promoting equity through payment model design, including requiring explicit plans by participants detailing how they will improve health equity; collection of data on social drivers of health; and use of financial incentives directly tied to equity.
