Elevance Health’s health services arm, Carelon, intends to purchase Nashville-based CareBridge, a value-based home and community-based care provider for patients with chronic and complex needs.

In an Oct. 17 call with investors, Elevance’s President and CEO, Gail Boudreaux, said the acquisition will serve as the foundation for Carelon’s home health business.
