The Centers for Medicare and Medicaid Services (CMS) have endorsed broad implementation of value-based care payment models and set aggressive goals for adoption by 2030. The most widespread and successful value-based model in Medicare has been the Medicare Shared Savings Program (MSSP), wherein groups of providers are accountable for the total cost and quality of care for a designated population of Medicare beneficiaries based on primary care attribution. CMS data indicate that MSSP accountable care organizations (ACOs) generate higher savings when the ACOs are composed mostly of primary care physicians; the most successful Center for Medicare and Medicaid Innovation (the Innovation Center) models have also built on top of primary care accountability.
