CVS Health is launching a platform to address social determinants of health as insurers roll out strategies to move into their communities and beyond the doctor’s office to reduce costs and improve outcomes.

CVS Health and its Aetna health insurance unit say they will collaborate with a “social care coordination platform” called Unite Us as part of a coming series of initiatives the healthcare giant is rolling out to address social determinants of health. Financial terms of the partnership aren’t being disclosed.
