Community health leaders, researchers, and others have long recognized the influence that social, economic, and structural factors can have on health and well-being. Despite evidence that addressing health-related social needs (HRSNs) and integrating social and medical services can improve health and reduce costs, social service and medical systems often operate in silos, resulting in fragmented and inefficient care.
These silos are, in large part, created by separate payment systems with different requirements and misaligned incentives. High levels of spending on health care services may “crowd out” investment in social services, which have long struggled to garner sufficient funding to meet need.
