What the 2026 Physician Fee Schedule Means for RPM and RTM

The 2026 Medicare Physician Fee Schedule final rule came out on November 5th, 2025, and buried within its hundreds of pages lies a significant evolution in how CMS approaches remote patient monitoring (RPM) and remote therapeutic monitoring (RTM). For those of us who’ve watched these codes mature from their tentative introduction to mainstream adoption, this year’s rule signals that CMS is grappling seriously with a fundamental question: how do you pay for technology-enabled care in a fee-for-service system designed for discrete face-to-face encounters?
