Value-based care management company AllyAlign Health last week announced it will offer two new co-branded Medicare Advantage plans to members of LeadingAge California, thanks to a partnership with the senior advocacy state group.

The plans — institutional equivalent special needs plan (I-SNP) Align Thrive, and chronic conditions special needs plan (C-SNP) Align Connect — will be underwritten by California licensed Knox-Keane Healthcare Service Plan Align Senior Care and serve long-term care residents living with complex, chronic health needs as well as those diagnosed with dementia.

Richmond, Va.-based AllyAlign partners with long-term care providers to set up Medicare Advantage I-SNPs, C-SNPs and dual-eligible special needs plans (D-SNPs). The company currently operates in more than 550 communities across 22 states.
