Home Health lives or dies on the quality of its Outcome and Assessment Information Set (OASIS) documentation.

OASIS is the standardized assessment tool that every Medicare-certified home health agency must complete at patient admission, recertification, transfer, and discharge. OASIS captures hundreds of data points—from wound status and mobility to cognitive function and medication management. OASIS also determines both patient outcomes and reimbursement levels under the Patient-Driven Groupings Model (PDGM).

In other words, OASIS is the backbone of clinical, financial, and compliance performance in home care.
