Diagnostic errors represent one of the most costly and prevalent types of medical error occurring across inpatient and ambulatory care environments. Efforts to reduce these error rates have faced numerous challenges, and little progress has been made compared to other areas in safety. Critical to making an accurate diagnosis are the physician’s cognitive processes such as the ability to find and comprehend relevant information, formulate a diagnosis, and then track information to ensure that appropriate follow-up occurs.
