A New Era for Technology-Enabled Chronic Care in Medicare

The Centers for Medicare & Medicaid Services Innovation Center has introduced the ACCESS Model—Advancing Chronic Care with Effective, Scalable Solutions—a ten-year voluntary national model testing whether an alternative payment methodology can reduce Medicare expenditures while preserving or enhancing quality of care. Running from July 5, 2026 through June 30, 2036, the ACCESS Model introduces Outcome-Aligned Payments (OAPs) for technology-enabled chronic care, representing a fundamental departure from traditional fee-for-service payment structures that have historically created barriers to innovative care delivery.
