Payers fined by states in 2025:
- Regence BlueShield was fined $650,000 by Washington state in September for violations related to the Mental Health Parity and Addiction Equity Act. The company was also fined $100,000 by the state in October 2025 for improperly denying 954 claims that should not have required prior authorization.
- Cigna was fined $500,000 by California in September after an investigation found the insurer violated state utilization review laws by denying claims as “not medically necessary” without a physician exercising clinical judgment.
