As the Affordable Care Act (ACA) celebrates its 12th anniversary, President Joe Biden has proposed fixes for some of its most glaring shortcomings, most notably the family glitch in eligibility for premium subsidies. However, he and congressional Democrats have cooled to one of the more promising ideas for reform: creating a public option to compete with private plans in the Marketplaces. Such a plan would offer benefits similar to Medicare and could promote more robust competition for plan quality and cost, especially in areas with fewer Marketplace choices. Concerns about the potential cost and complexity of the public option generated substantial opposition that muted initial support.
