Two years ago next month, acting administrator of the Centers for Medicare and Medicaid Services (CMS), Andy Slavitt, announced that the agency would begin implementing the Merit-based Incentive Payment System (MIPS), authorized under the 2015 Medicare Access and CHIP Reauthorization Act (MACRA), by allowing eligible clinicians in 2017 to “pick your pace.” The composite performance score, or the score needed to avoid a subsequent year negative Medicare Part B payment adjustment, was set at a nominal three out of 100 points. This year, the composite performance score is 15 points. This past February, Congress, under the Bipartisan Budget Act, modified MACRA to allow CMS to further delay MIPS implementation through 2022.
