The social determinants of health program will leverage patient engagement tools that refer patients to social services and do patient outreach.
RWJBarnabas Health (RWJBH) has unveiled Health Beyond the Hospital (HBH), a program codifying the healthcare organization’s commitment to addressing the social determinants of health.
The program, powered by technology from social determinants of health referral platform NowPow and patient engagement technology ConsejoSano, will include patient screening, referral to social services, and ongoing patient outreach and engagement outside the facility.
This comes as the healthcare industry increasingly focuses on the social determinants of health, or the social factors that limit a patient’s ability to obtain and maintain health and wellness.
