Humana’s Bold Goal project is continuing to bear fruit for individuals with chronic conditions as the payer focuses in on addressing the social determinants of population health.

The ongoing initiative is increasing the number of self-reported “healthy days” among Medicare Advantage members after Humana added social determinants screenings to their routine protocols.

Humana uses a four-question survey to assess a patient’s physical and mental capacity to function at their highest level, allowing the payer to understand when health concerns interfere with daily functioning during “unhealthy days.”

The 2019 report found that the efforts have reduced the number of unhealthy days in participating members by 2.7, while MA beneficiaries in non-participating communities have seen an increase of 0.6 unhealthy days each...
