As more Medicaid agencies embrace value-based care, they must reconsider strategies to address the social determinants of health.

As state Medicaid programs look to treat more patients for the social determinants of health (SDOH), they are looking to retool their intervention efforts for a next generation, according to a new report from Manatt.

These efforts come as Medicaid programs aim to cut costs and reimburse more for care outcomes. These state programs know they must address the social factors that can account for up to 80 percent of population health, but that it will require revamped screening policies and community health outreach.
