There was a time, not even very long ago, when reference to our alleged “healthcare” system as a “disease care” or “sick care” system seemed both insightful and clever, and even surprising. Now, such commentary is routine enough to seem cliché. The allusions remain apt, however, despite the patina of increasingly frequent repetition.

We have a vast system of disease care systems—hospitals, clinics, extended care facilities, pharmacies, and so on—and virtually no system at all for the basic care of health. As fate would have it, “virtually” proves to be a portent, as well as a qualifier.
