In October 2018, the Centers for Medicare and Medicaid Services (CMS) doubled down on bundled payments through its launch of Bundled Payments for Care Improvement Advanced (BPCI-Advanced). In a prior post, we described ways in which BPCI-Advanced reflects progress in bundled payment policy by comparing and contrasting the program to its recently concluded predecessor, Bundled Payments for Care Improvement (BPCI)“Classic.” Built upon the design principles of model 2 in the Classic program, BPCI-Advanced holds clinicians and provider organizations accountable for quality and costs of care across a defined episode comprising either a hospitalization or procedure and 90 subsequent days.

Early enrollment in BPCI-Advanced has been robust, with 1,547 physician groups and hospitals volunteering to join the program. This strong...
