Telehealth company Amwell is partnering with digital chronic condition management platform DarioHealth to add a cardiometabolic program to its suite of digital offerings.

Under the partnership, patients who receive care from clinicians using Amwell’s telehealth platform can be referred to Dario’s program for chronic conditions like diabetes, high blood pressure and weight management.

The digital program uses connected devices, like glucometers, scales and blood pressure monitors, as well as data tracking and support from health coaches. It will be available through Amwell in the second half of the year.
